Forms

UPDATED 12/06/11: Forms will be available in June 2012.

Here are the forms you will need for this year's Quaker Rowing Camp. Please print, fill out, and bring each to registration the first day of camp. If you are an overnight camper, that will be Monday from 4-6:00 PM at the Rodin Dorm (see directions for location). If you are a day camper, please be at the boathouse between 8-9:00 AM on Tuesday of each week. We are located at the Upenn Boathouse at #11 Kelly Drive.

**Overnight campers please bring a check for the room key deposit in the amount of $160 made out to the Trustees of the University of Pennsylvania. This check will be returned to you at the end of camp as long as the key is not lost.

The forms for the 2012 Quaker Rowing Camps will be not be available until June 2012. Forms are to be completed and brought to registration on the first day of camp. The forms include a participant agreement form, a medical treatment consent form, and a parent permission form. We do not require a physical form. Do not mail this material to us in advance. We ask that if you are arriving by plane and wish to use the QRC shuttle described on our transportation page that you e-mail us with travel details (airline, flight number, and arrival time).

We will send out a final e-mail approximately three weeks before the start of your specific camp. This e-mail will include arrival/departure instructions as well as directions to the campus dormitories and the boathouse.

Please make sure to keep record of your username and passcode to access your online account. We do not require signed medical forms from a family physician.

Once again, all forms will need to be brought to registration on the first day of camp. Forms will not be accepted via regular mail.
